What Is a DOT SAP Evaluation?

A DOT SAP evaluation is a required process for employees in safety-sensitive positions who have violated DOT drug or alcohol regulations. Conducted by a qualified Substance Abuse Professional, the evaluation determines your readiness to safely return to duty.

This process ensures both public safety and employee well-being—it's not about punishment, but about supporting recovery and accountability.

🛠️ What to Expect: Key Steps

1. Initial Assessment

Your evaluation begins with a comprehensive assessment:

  • Personal interview
  • Review of medical/treatment records
  • Standardized questionnaires or screenings
  • $250

This helps us understand your history, current needs, and build a plan that supports your return to work.

2. Personalized Treatment Plan

Based on your assessment, we’ll create a tailored plan that may include:

  • Education or awareness classes
  • Counseling (individual or group)
  • Outpatient or inpatient treatment
  • Follow-up testing and monitoring

Our goal: to support your recovery and ensure you're fit to return to your role.

3. Follow-Up Evaluation

Once you’ve completed your treatment plan, we’ll conduct a follow-up assessment to:

  • Review your progress
  • Confirm compliance with the SAP recommendations
  • Determine if you're ready for safety-sensitive duties
  • $250

This step is crucial to completing the Return-to-Duty process.

🧾 What to Bring to Your Evaluation

To help things go smoothly, please bring:

  • A valid photo ID
  • Violation documentation (e.g., test results or employer letter)
  • Medical or treatment records
  • Employer and provider contact info
